Treatment of autonomic vascular dysfunction – what’s the problem?

The symptoms of the AVD are presumably due to low stability of the heart and / or the cardiovascular system as a whole, often manifested only in stressful situations, but possible without them, due to the disruption of the autonomic nervous system with homeostasis and functional disorders a number of cases supplying blood to the brain. Thus, there is reason to believe that psycho-emotional disorders should be considered as secondary conditioned neurosis-like states. The AVD can also be diagnosed as psycho-vegetative syndrome, autonomic dysfunction syndrome, neurocirculatory dystonia, neurocirculatory asthenia or vegetative neurosis. Such diagnoses are usually made by therapists and according to some sources, they put them in 25–70% of patients. Interestingly, such a diagnosis exists only on the territory of the CIS – there is no such disease in any international classification!

You will be surprised at the multiplicity of symptoms for which you can be diagnosed. In patients with AVD, about 150 symptoms and 32 syndromes of clinical disorders are described. The most common symptoms of AVD are cardialgia, asthenia, neurotic disorders, headache, sleep disturbances, dizziness, respiratory disorders, rapid heartbeat, cold hands and feet, vegetative-vascular paroxysms, hand tremors, internal tremors, cardiophobia, myalgia, pain in the joints swelling of tissues, heart failure, fever in the face, subfebrile, orthostatic hypotension, syncope.

The most common signs are:

– cardialgia;
– cardiopalmus;
– vascular dystonia;
– autonomic dysfunction;
– respiratory disorders;
– systemic neurotic disorders.

Dysuric events that contribute to the occurrence of urolithiasis are possible, just as biliary dyskinesia is a risk factor for gallstone disease.

As you can see, these symptoms are very diverse, some of them are opposite. They do not belong to any particular organ, they are not identified with any disease. The number of complaints is also striking.

Also interesting is the whole process of diagnosis of autonomic dysfunction. Usually, the doctor prescribes quite expensive methods of neuroimaging – MRI, tomography of the brain, ultrasound of cerebral vessels … But all this he prescribes in order not to find anything serious! Indeed, autonomic dysfunction is a disease that is characterized by the absence of organ damage, and if the doctor sees any serious violations on the results of the examination, then in no case will he diagnose autonomic disorder.

It turns out that the doctor diagnoses the AVD in the case when it can not make a different diagnosis!

Causes of AVD

Despite the mystery of the diagnosis of “vascular dystonia”, patients complain of real suffering. What is the cause of this suffering?

All processes in our body are regulated by two nervous systems – somatic and autonomic, or autonomous.

The somatic nervous system carries out our will – muscles are subordinated to it. With the help of it we move. The autonomic nervous system is subject to all those processes of the body that take place as if “by themselves”, without our conscious participation – digestion, blood circulation, metabolism, maintaining body temperature.

The higher centers of the autonomic nervous system are closely related to our emotions – and this is not without reason – because each emotion causes certain changes on the part of internal organs – which is reflected even in sayings: the heart jumps for joy, the legs for fear rub off, for sadness tears flow, going on from fright – we will not remember.

Without a doubt, the suffering of patients with vascular dystonia comes from a violation of the regulatory functions of the autonomic nervous system, and their main cause is psycho-emotional factors – the long experience of strong, mostly negative emotions.

Alexander Moiseevich Vein (1928 – 2003), a well-known researcher of diseases of the autonomic nervous system, professor, academician of the Russian Academy of Medical Sciences, wrote that acute or chronic emotional stress is the cause of vascular dystonia. And this point of view is shared by the absolute majority of specialists.

So, the cause of vegetative dystonia is a psycho-emotional disturbance. Therefore, it must be treated by specialists in the field of the psyche. Why, in practice, neuroscientists and therapists treat vascular dystonia?

Ukraine is not Europe and not America, where it is prestigious to go to a psychotherapist and a psychiatrist.

Mental difficulties are usually regarded as something shameful, they must be hidden and not shown to anyone – well, except that you can cry in your best friend or buddy. And in no case can not show your heartache to the doctor! With the same physical pain, it is customary to go to doctors. So the patients come to therapists and neurologists, and they do not understand what they are complaining about, without mentioning (and not realizing) what is in their hearts.

And the therapist or neurologist cannot say to the patient: “No, this is not mine. Go, my friend, to a psychiatrist. ” And if they say, what will the reaction of our average patient be? “I have a headache for him, and he is going to see me to a psychiatrist?” – and the patient will not go to a psychiatrist, and will not receive adequate treatment.

What is the problem?

AVD from the perspective of a non-psychiatrist:

Therapists and neurologists prescribe, as a rule, the following groups of drugs: drugs that expand the brain vessels (cavinton, cinnarizine, stugerone, sermion), nootropics, vitamins, metabolic drugs (ceraxone, actovegin), drugs that affect heart activity. Most of these drugs stop the symptoms of vegetative dystonia by strengthening the overall tone of the body and, in particular, the nervous system, thus giving the body more resources to resist. Other drugs act on the organs themselves, forcing them to work “as it should,” but at the same time they also have no influence on the treatment of the causes of the AVD.

It cannot be said that the treatment carried out by neurologists and therapists does not help. It removes the symptoms, but does not remove the cause of the disease. Therefore, such treatment should be carried out for life. During treatment interruptions, symptoms return. In addition, some treatment methods eventually stop working due to the body’s addiction to drugs, and the further development of the disease process (after all, the cause is still present). Sometimes there is a shift in symptoms: the headache, after treatment the head stopped hurting, but the hands became cold and weakness appeared.

Due to the long-term use of drugs, their side effects naturally accumulate, which does not have the best effect on the body as a whole. Also, with long-term treatment of symptoms of vegetative dystonia without appropriate treatment of its cause, psychosomatic diseases, such as a stomach ulcer, hypertension, migraine, allergies, bronchial asthma, obesity, are formed rather quickly. In addition, the actual psychiatric diseases may manifest themselves: neurosis, depression, phobias, obsessions. True, lately, neurologists are increasingly prescribing “psychiatric” drugs – tranquilizers and antidepressants. On the one hand, this is good. On the other hand:

These specialists do not diagnose autonomic dysfunction. Because they do not recognize that such a diagnosis exists. From the point of view of psychiatrists and psychotherapists, vascular dystonia is the maximum syndrome. In this, psychiatrists and psychotherapists in the CIS fully share the worldview of doctors around the globe – we recall that the diagnosis of vascular dystonia is in circulation only among CIS doctors – and now it’s clear why.

In other countries, patients do not hesitate to go to a psychotherapist, and doctors of other specialties do not pretend to invent non-psychiatric diagnoses.

In patients with a diagnosis of vegetative-vascular dystonia, psychotherapists in most cases diagnose the following diseases:

  • Nervous exhaustion, or neurasthenia – is able to give the whole range of symptoms that characterize vegetovascular dystonia.
  • Depression. It is also manifested by many symptoms observed in vascular dystonia. Especially often the latent depression gives a clinical picture of vegetative-vascular dystonia, since with it a person does not feel heartache – the body suffers for the soul.
  • Reaction to stress. According to doctors of all specialties, it is stresses that are the main and main cause of vegetative-vascular dystonia.
  • Panic attacks are manifested by a sudden feeling of anxiety, fear, and numerous autonomic disorders: dizzy, heavy and numb legs, hands, heartbeat quickens, a person feels pre-marrow.
  • Phobias – fears of certain places and situations. Fear is manifested by multiple vegetative manifestations.
  • Anxious-miserable accentuation of the personality – people with anxious accentuation of the personality takes everything to heart, are worried about every little thing, are always worried.
  • Anxiety Disorder – A person with anxiety disorder constantly experiences anxiety, and as a result, disorders of the autonomic nervous system appear.

Treatment of vascular dystonia

Proper treatment of vegetative dystonia is the elimination of its cause: the normalization of the emotional state of the patient.
